Thailand doesn't have a major drug problem anymore. Now it just has a major problem with drugs. Generic pharmaceuticals, Generic drugs are now headline news in Thailand. That's quite a sweeping and confusing statement, let me explain. The ex Thai Prime minister enforced a serious crackdown in the later part of his 'reign' on drug dealers and drug traffickers as well as the drug users themselves. This led to a let us say a 'culling' of the dregs of Thai society. Some call it an extra judicial killing spree. At the end of this Taksin declared that the streets of the capital and every small town and village in the Kingdom of Thailand was now drug free. A little over the top maybe in every way but he certainly made a 'difference'

Nowadays there is a different problem with drugs, Fake drugs, copy drugs. Not fake ecstasy, or Yaa Baa, as it is called here in Thailand. Sorry was called here in Thailand. I forgot the streets are clean now. But I still see reports of drug arrests on Thai TV - ah well it is all a bit confusing.

So the problem now is the legal copying of drugs originally patented by western pharmaceutical companies. Or should that be the illegal copying of drugs originally patented by western pharmaceutical companies. I suppose that depends on whether you are Thai or not Thai.

Generic drugs can be obtained at a fraction of the cost of 'brand name' drugs. However, only in pharmacies outside major hospitals.

Commonly used drugs are widely available throughout Bangkok. For sure, do not be surprised if a particular drug, just released in U.S.A. or Europe, is not yet available. Up to about one year ago, it was possible to just get about anything without prescription, including antibiotics, sedatives, anti-depression medication, recreational drugs like Viagra etc. In many cases this may not have been strictly according to the law, but in practice there were no obstacles in obtaining what you wanted. Lately, it seems to be rather difficult to get sedatives, so if you suffer from anxiety when going to the dentist and need some comfort, you will have to go to one of the hospitals and see a doctor there first. We do think that recreational drugs still can be obtained easily. [It is recommended you enter a pharmacy when there is nobody else inside, so the pharmacist does not have to take other customers into account, when dealing with you]

As said, pharmacies are abundant. There is one pharmacy chain around town, called Fascino. They seem to have the most competitive prices, and operate strictly legal (that is they will not give you anything without a prescription, if one is needed). Unfortunately, they do not have a lot of outlets.
There is occasional talk about 'fake' medicine. We think this is largely overblown, and that medication not obtained in some obscure off the road pharmacy in the middle of nowhere, is reliable.

Another interesting issue is the use of generic drugs. These actually are widely available at outside pharmacies at a fraction of the cost of 'brand name' drugs. When going to a hospital however, you invariably will be provided with the expensive variety. We do recall however one day getting some cheap generic paracetamol at a reputable hospital. Just maybe one day, hospital pharmacies could put their heads together and make a list of reliable generic drugs available in Thailand (many actually produced locally) and start using them and having them prescribed by their doctors.

As an example we can quote simvastatin (well known under the name Zocor). While the imported (original) version costs up to 50 baht a tablet, it is also possible to buy the locally produced version at 42 baht for 10 tablets of 10 mg, under de brandname Zimmex, produced by Silom Medical Co. Ltd.) So you can get this medication at about 10% of the price of the original!!!
Now if the pharmacist sees you coming, he or she will certainly want to sell you the expensive medication first, so you have to be prepared and ask for the brandname mentioned above. The same applies for other drugs, just ask for the locally made product, and ask in different pharmacies!

But what of the health minister's claim to lack the money to purchase brand-name drugs? While Thailand is not wealthy by Western standards, it is not poor either. With a 2006 per-capita GDP of $9,100[10] and the second-largest economy in Southeast Asia, Thailand can afford to pay more for AIDS medicines than most countries in the region and many countries in other parts of the world. The AIDS epidemic in Thailand, largely fueled by its notorious sex industry, is on par with Brazil's.[11] There are approximately 500,000 HIV-positive people in Thailand out of a total population of 64.6 million.[12]

Middle-income countries with large HIV-positive populations have led the charge to secure cheaper drugs by using strong-arm tactics against Western pharmaceutical companies. What started with Brazil and continued in India[1] has now spread to Southeast Asia. Thailand is going further than Brazil ever did and is breaking patents on a heart disease drug as well as HIV drugs. Thailand claims that this action is to support its goal of making high-quality medicines available to patients on a sustainable basis, but it has lowered its health budget, belying this claim. The damaging practice of breaking patents will harm Thai patients as the research-based industry withdraws from the country, resulting in less investigation of drugs that respond to resistant strains of diseases. It will also harm patients worldwide, as lower revenues for drug companies mean less funding for development of new drugs and fewer incentives to introduce new drugs in the developing world.
